Registrar's Forms

Request Forms

The Office of the Registrar will mail forms maintained by our office to the colleges and departments upon request.

Registrar Forms

  • E-mail your request to reg-forms@listserv.arizona.edu
    • Include your name, department/college name, campus address, which form you are requesting, and the quantity.
  • Requested forms will be sent via campus mail with the exception of the Change of Grade form and forms available online.

Forms furnished by mail:

  • Administrative Drop/Reinstatement 
  • Complete Withdrawal (mailed to the Dean of Students Office only)
  • General Petition
  • Petition for Completion of Coursework
  • Undergraduate Student Leave of Absence

Change of Grade Form:

  • Change of Grade forms are only available for in-person pick up by faculty and staff with photo ID. Students, including student employees, are not permitted to pick up the Change of Grade form.
