Registrar Forms

The Office of the Registrar will mail forms maintained by our office to colleges and departments upon request.

Please e-mail your request to reg-forms@listserv.arizona.edu

  • Include your name, department/college name, campus address, which form you are requesting, and the quantity.

Requested forms will be sent via campus mail with the exception of the Change of Grade form and forms available online.

Forms furnished by mail:

  • Complete Withdrawal (mailed to the Dean of Students Office only)
  • General Petition
  • Petition for Completion of Coursework
  • Undergraduate Student Leave of Absence

Change of Grade Form

  • Change of Grade forms are only available for in-person pick up by faculty and staff with photo ID. Students, including student employees, are not permitted to pick up the Change of Grade form.
